Understanding rheumatoid arthritis

Rheumatoid arthritis (RA) is a chronic autoimmune disease in which the immune system, which normally protects the body from infection, attacks its own tissues, mostly the joints. First-line treatment

According to the 2021 American College of Rheumatology (ACR) guideline for the treatment of rheumatoid arthritis, methotrexate—a conventional disease-modifying antirheumatic drug (DMARD)—is the preferred initial treatment for DMARD-naive patients with moderate-to-high disease activity. The guideline strongly recommends methotrexate monotherapy over hydroxychloroquine or sulfasalazine, and over biologic or targeted synthetic DMARDs, generally starting with methotrexate alone rather than combination therapy. The guideline also strongly recommends a "treat-to-target" approach—systematically monitoring and adjusting therapy to reach low disease activity or remission—and strongly advises against longer-term (three months or more) glucocorticoids.
