Understanding adrenal insufficiency
"Adrenal insufficiency" is a category, not one disease. For diagnosis it suggests the corticotropin (ACTH) stimulation test over other tests, and — where that test is not feasible — a low morning cortisol (under 5 µg/dL) combined with ACTH as a preliminary signal. For replacement it suggests hydrocortisone or cortisone acetate in two or three divided oral doses per day, with the largest dose in the morning at awakening, and advises against dexamethasone because of Cushingoid side effects from difficult dose titration. Where aldosterone deficiency is confirmed, it recommends fludrocortisone and that patients not restrict salt. It also recommends every patient carry a steroid emergency card, medical alert identification, and a glucocorticoid injection kit they have been taught to use. NIDDK adds that corticosteroid doses need adjusting during illness, and must be increased with a high fever.
