Understanding hypogonadism

Hypogonadism is an umbrella term covering both sexes: MedlinePlus (NLM) defines it as when "the body's sex glands (gonads) produce little or no hormones." So the category holds men with testicular or pituitary failure, women and girls with ovarian failure, and children with delayed puberty — and MedlinePlus states that "Menopause is the most common reason for hypogonadism," which is why the term is far broader than the "low T" content that dominates search results. Where the failure sits changes what it means. StatPearls (NCBI Bookshelf) describes primary (hypergonadotropic) hypogonadism as "persistently low testosterone measurements with above-normal LH levels," and secondary (hypogonadotropic) hypogonadism as "low serum testosterone with normal or low LH levels" — a hypothalamic or pituitary problem. That is why MedlinePlus lists FSH and LH among the tests ordered, not the sex hormone alone: an identical low hormone value can mean testicular or ovarian disease in one person and pituitary disease, sometimes a tumor, in another. It is also not diagnosed from one blood draw. StatPearls notes hypogonadism "is usually diagnosed when the morning serum testosterone level is <300 ng/dL on at least 2 occasions," alongside symptoms. This is general information, not medical advice; decisions belong with your clinician.

First-line treatment

The 2018 Endocrine Society clinical practice guideline on testosterone therapy recommends "diagnosing hypogonadism in men with symptoms and signs of testosterone deficiency and unequivocally and consistently low serum total testosterone" — and one low reading does not settle it, because the guideline also recommends "confirming the diagnosis by repeating the measurement of morning fasting total T concentrations." It recommends against routine screening of men in the general population, and recommends distinguishing primary from secondary hypogonadism "by measuring serum luteinizing hormone and follicle stimulating hormone concentrations" — a step that comes before any prescription. The same guideline recommends against testosterone therapy in men with "breast or prostate cancer, a palpable prostate nodule or induration." For girls and women, MedlinePlus (NLM) says estrogen and progesterone are used. The FDA states that "None of the FDA-approved testosterone products are approved for use in men with low testosterone levels who lack an associated medical condition" — though the agency's position on aging has shifted: FDA says that in June 2026, after the TRAVERSE cardiovascular safety trial, it requested (not yet completed) label updates removing the limitation of use stating that the safety and efficacy of testosterone therapy in men with age-related hypogonadism had not been established.
