Two SSRIs that are more alike than the internet pretends. They carry the same boxed warning and the same core contraindications; one place their labels clearly differ is the list of conditions each is approved to treat.
The key difference
One difference that matters is the approved indication. Zoloft's label lists major depressive disorder, OCD, panic disorder, PTSD, social anxiety disorder and PMDD — and in children, only OCD (ages 6-17). Lexapro's lists major depressive disorder (12 and older) and generalized anxiety disorder (7 and older). The diagnosis, not a potency ranking, is what points a prescriber to one of these.
Both carry the same boxed warning: antidepressants increased the risk of suicidal thoughts and behaviors in pediatric and young adult patients in short-term studies, and anyone starting one should be watched closely.
Both are contraindicated with MAOIs — including linezolid and IV methylene blue — and within 14 days of stopping one, and both with pimozide. That MAOI rule exists because of SEROTONIN SYNDROME, which both labels say can occur with other serotonergic drugs but also on the SSRI alone. Both also warn of DISCONTINUATION SYNDROME: stopping abruptly can bring nausea, dizziness, electric-shock sensations and insomnia, so the dose is tapered.
What favours Lexapro (escitalopram)
Lexapro's label is the one that names generalized anxiety disorder, and it is the one approved down to age 7 for GAD (12 and older for depression). If GAD is the working diagnosis — particularly in a younger patient — that is a labelled reason a prescriber may reach for escitalopram.
What favours Zoloft (sertraline)
Zoloft's label covers the wider spread: OCD, panic disorder, PTSD, social anxiety disorder and PMDD alongside depression. It is also the one approved for OCD in children from age 6. A diagnosis outside depression and GAD is the labelled reason a prescriber may reach for sertraline.
Bottom line
If you are being treated for depression, these two are far closer than the comparison articles suggest — same class, same boxed warning, same MAOI and pimozide contraindications, same serotonin-syndrome and discontinuation warnings. The choice belongs to your prescriber, who weighs your diagnosis, your other medicines and your history against the label. Two things are worth carrying into that conversation: tell them every other serotonergic drug or supplement you take, and do not stop either one on your own — ask how to taper.
