What Vitamin D3 + K2 is
Vitamin D3 + K2 is a combination supplement that pairs cholecalciferol (vitamin D3), a fat-soluble vitamin the body also makes from sunlight, with menaquinone (vitamin K2, typically the MK-4 or MK-7 form). What the evidence shows
For bone, the NIH notes that clinical trials have shown vitamin D with calcium may increase bone mineral density in some postmenopausal women and older men; the evidence on whether the two reduce falls and fractures is mixed (some trials and a large meta-analysis suggest a modest reduction in fracture risk, while other reviews and the USPSTF found no benefit on falls or fractures in community-dwelling older adults), and effects on muscle strength have been inconsistent. For vitamin K, the NIH states it is unclear whether supplementation with any form reduces the risk of osteoporosis, and that only some studies link higher intake to higher bone density. Cardiovascular and arterial-calcification claims rest mainly on observational data and a biological hypothesis (the role of vitamin K-dependent matrix Gla-protein) rather than clinical proof, so the efficacy of a combined D3+K2 product is not established.
Typical dosage
The NIH Recommended Dietary Allowance for vitamin D is 15 mcg (600 IU) per day for adults aged 19 to 70 and 20 mcg (800 IU) for those over 70; the Adequate Intake for vitamin K is 120 mcg/day for men and 90 mcg/day for women. There is no NIH-established benefit to any particular "D3+K2" ratio, and many commercial products contain doses well above these reference intakes. Safety

The Tolerable Upper Intake Level for vitamin D is 25 to 100 mcg (1,000 to 4,000 IU) per day depending on age (100 mcg/4,000 IU for adults), and combining vitamin D with calcium supplements may raise the risk of some adverse effects. Vitamin K, by contrast, has no established Upper Intake Level because of its low potential for toxicity, but anyone on blood thinners should be especially cautious about the K2 component and talk to their provider first.
Side effects
Per the NIH, the Food and Nutrition Board found no adverse effects from vitamin K consumed in food or supplements and so set no Upper Intake Level. Side effects from the D3 component arise mainly from taking too much: excess vitamin D causes hypercalcemia, which the NIH says can produce nausea, vomiting, muscle weakness, loss of appetite, dehydration, excessive thirst and urination, and kidney stones.
Interactions & cautions
The most important interaction is with the vitamin K2 component: the NIH describes a serious and potentially dangerous interaction between vitamin K and anticoagulants such as warfarin (Coumadin), which antagonize vitamin K, so people taking them must keep their intake consistent and consult their provider. The NIH also notes that the vitamin D component can interact with orlistat, statins, corticosteroids, and thiazide diuretics (the last especially when combined with calcium, which may raise the risk of hypercalcemia), and that prolonged antibiotic use may affect vitamin K status.
