How long does Topiramate stay in your body?
The elimination half-life of topiramate is about 21 hours — the time it takes the body to clear about half of it. As a rule of thumb, a medicine is mostly gone after roughly 4 to 5 half-lives, though this varies from person to person with age and kidney or liver function. Linear, dose-proportional kinetics — no active metabolite outlasts the parent (topiramate is largely excreted unchanged in urine, ~70%) and it is not a prodrug. Steady-state is reached in about 4 days with normal renal function. The half-life is prolonged in kidney impairment: plasma clearance falls ~42% in moderate renal impairment (and dose is halved for creatinine clearance <70 mL/min), so the drug clears more slowly. It is modestly longer in older adults (half-life ~13% longer, clearance ~20% lower), and clearance drops ~26% in moderate-to-severe hepatic impairment.
