Understanding cachexia

Cachexia is the wasting that accompanies serious chronic illness, and the category is broader than most people assume: StatPearls (NCBI Bookshelf) describes it not only in cancer but in "chronic obstructive pulmonary disease (COPD), chronic heart failure, chronic kidney disease, and chronic infectious and inflammatory diseases including AIDS." StatPearls defines it as "a complicated metabolic syndrome related to underlying illness and characterized by muscle mass loss with or without fat mass loss," linked to inflammation, insulin resistance and increased protein turnover. It is defined by thresholds, not by appearance. StatPearls gives diagnostic criteria of 5% weight loss in 12 months, or a body mass index under 20 kg/m2, in a person with a known chronic disease — plus at least 3 further factors from a list that includes loss of muscle mass, asthenia, and loss of body fat, with inflammation evidenced by albumin under 3.2 g/dL or increased C-reactive protein. It also stages the process: pre-cachexia (weight loss over 1 kg but under 5%), cachexia (weight loss over 5%, or over 2% with BMI under 20), and refractory cachexia (weight loss over 15% with BMI under 23, or over 20% with BMI under 27). The part people get wrong: eating more does not undo it. StatPearls states that "Increasing calorie–protein intake by definition does not reverse this abnormal metabolic state." The NCI PDQ nutrition summary reports that "increased caloric intake, whether by the oral route or by supplementation with total parenteral nutrition, has failed to counteract the wasting process," and cautions that "obesity may mask malnutrition" — a normal or high BMI can hide severe muscle depletion. This is general information, not medical advice.

First-line treatment

Management is defined by what is offered, not by a standard drug. The 2020 ASCO guideline on the management of cancer cachexia states that "Dietary counseling may be offered with the goals of providing patients and caregivers with advice for the management of cachexia," while "Enteral feeding tubes and parenteral nutrition should not be used routinely." On medication, ASCO names classes rather than a standard of care: "When it is decided to trial a drug to improve appetite and/or improve weight gain, currently available pharmacologic interventions that may be used include progesterone analogs and short-term (weeks) corticosteroids." ASCO's 2023 rapid recommendation update added a further option — "for adults with advanced cancer, clinicians may offer low-dose olanzapine once daily to improve weight gain and appetite." Weight and appetite are the stated targets; the underlying illness still drives the syndrome.
