PROZAC vs ZOLOFT

Both are SSRIs, both carry the same boxed warning, and for depression they are far more alike than the internet pretends. What separates them is on their labels — and the choice is your prescriber's.
The key difference
One difference that matters is how long fluoxetine stays in you. Prozac's label warns that fluoxetine and its active metabolite clear so slowly that a dose change "will not be fully reflected in plasma for several weeks," affecting both titration and withdrawal. Sertraline's mean terminal half-life is about 26 hours. You see it in the labels: after stopping Prozac you must wait 5 weeks before an MAOI; Zoloft's label says 14 days.
Both carry the same boxed warning — antidepressants increased the risk of suicidal thoughts and behaviors in children, adolescents and young adults; anyone starting either should be watched closely for worsening. Both can precipitate SEROTONIN SYNDROME, a potentially life-threatening condition, and that is why both are contraindicated with MAOIs, including linezolid and intravenous methylene blue. Both warn of a DISCONTINUATION SYNDROME — dizziness, electric-shock sensations, irritability, insomnia, and in sertraline's list, seizures — after stopping, especially abruptly. Neither should be stopped on your own; both labels ask for a gradual taper.
What favours Prozac (fluoxetine)
Prozac's approved uses include bulimia nervosa, which is not on Zoloft's label. Prozac is also approved in combination with olanzapine for depressive episodes in Bipolar I Disorder and for treatment-resistant depression — Prozac alone is not indicated for either. Its efficacy was shown in children as young as 7 for OCD and 8 for depression; Zoloft's pediatric approval is for OCD from age 6, and its label says effectiveness has not been established in children for anything else. A labelled indication is a reason; a preference is not.
What favours Zoloft (sertraline)
Zoloft's approved uses include PTSD, social anxiety disorder and premenstrual dysphoric disorder. None of those three appear on Prozac's label. Both are approved for major depressive disorder, OCD and panic disorder, so for those you are choosing between two drugs the FDA approved for the same job. Sertraline's shorter half-life is a labelled pharmacokinetic fact, not a promise of an easier time — it is why the wait before an MAOI is 14 days rather than 5 weeks. Zoloft's oral solution has its own contraindication (disulfiram) that the tablets do not.
Bottom line
If your prescriber is weighing these for depression, OCD or panic disorder, they are weighing two drugs the FDA approved for the same indication, with the same boxed warning and the same serotonin-syndrome contraindication. Labelled tiebreakers include: an indication only one of them carries, a drug you already take that interacts, and fluoxetine's long half-life. What you have read about "Prozac activates, Zoloft upsets your stomach" comes from separate trials that were never run head to head; their side-effect tables cannot be compared to each other.
