These two are not the same drug in different clothes: they work by different mechanisms and are approved for different lists of conditions. Which one fits you is a prescriber's judgement, not a coin toss.
The key difference
One difference that matters is what each is approved to treat. Zoloft (sertraline) is an SSRI, approved for major depressive disorder and also for OCD, panic disorder, PTSD, social anxiety disorder and PMDD. Wellbutrin XL (bupropion) is an aminoketone antidepressant, approved for major depressive disorder and for preventing seasonal major depressive episodes — its label carries no anxiety-disorder indication.
Both labels open with the same boxed warning: antidepressants increased suicidal thoughts and behaviour in children, adolescents and young adults; anyone starting either should be watched closely for worsening or emerging suicidal thoughts, and families told to watch too. Both are contraindicated with MAOIs, including linezolid and intravenous methylene blue, and within 14 days of stopping one — for sertraline because of SEROTONIN SYNDROME, for bupropion because of hypertensive reactions.
Sertraline's label also names DISCONTINUATION SYNDROME: stopping a serotonergic antidepressant abruptly can cause its own reactions. Taper with your prescriber; do not just stop.
What favours Wellbutrin XL (bupropion)
Bupropion is the only one of the two whose label covers prevention of seasonal depressive episodes, and it is not a serotonergic antidepressant. But its label rules it out entirely for some people: it is contraindicated if you have a seizure disorder, if you have a current or prior diagnosis of bulimia or anorexia nervosa, or if you are abruptly coming off alcohol, benzodiazepines, barbiturates or antiepileptic drugs — all because bupropion can cause seizures. Its label also warns it can raise blood pressure.
What favours Zoloft (sertraline)
Sertraline is the one with the anxiety-spectrum approvals — OCD, panic disorder, PTSD, social anxiety disorder — plus PMDD, alongside depression. The trade is the SSRI warning set on its own label: serotonin syndrome, increased bleeding risk (higher with aspirin, NSAIDs or anticoagulants), hyponatremia, QTc prolongation, sexual dysfunction, and the discontinuation syndrome above. Sertraline is contraindicated with pimozide, and the oral solution is contraindicated with disulfiram because it contains alcohol.
Bottom line
Unlike most "X vs Y" antidepressant pairs, these two genuinely are different animals — different mechanism, different approved conditions, different contraindications. That still does not make either one "better". The choice turns on your diagnosis and your history (seizures, an eating disorder, and abrupt withdrawal from alcohol or benzodiazepines take bupropion off the table; an anxiety diagnosis is on sertraline's label and not bupropion's), and it belongs to the person who can see your chart. Both carry the same boxed warning about suicidal thoughts and behaviour, and neither should be started, switched or stopped on your own.
