These two are not interchangeable, and the internet treats them as if they were. Their FDA-approved uses do not overlap, so the condition you actually have — not a potency ranking — decides this.
The key difference
One difference that matters is what each label is approved to treat. Modafinil improves wakefulness in adults with excessive sleepiness from narcolepsy, obstructive sleep apnea, or shift work disorder. Vyvanse treats ADHD (adults and children 6 and older) and moderate-to-severe binge eating disorder in adults. Modafinil for ADHD is not an FDA-approved use.
Vyvanse carries a boxed warning for ABUSE, MISUSE, AND ADDICTION: high potential for abuse, and misuse of CNS stimulants can cause overdose and death — a risk that rises with higher doses and with snorting or injecting. Its label says to avoid use in serious cardiac disease and to monitor blood pressure and pulse.
Modafinil has no boxed warning, but its own label says it produces euphoric effects consistent with other scheduled stimulants, tells physicians to watch for dose escalation and drug-seeking, and to consider extra monitoring in known cardiovascular disease. "No boxed warning" is not "no abuse potential."
What favours Modafinil (Provigil)
Modafinil is the labelled option for excessive daytime sleepiness from narcolepsy, sleep apnea or shift work — once daily in the morning, or about an hour before a shift. In sleep apnea it treats the sleepiness, not the obstruction: if CPAP is the right treatment for you, the label says to make a maximal effort with CPAP before and during modafinil.
Two things to raise with a prescriber: it can reduce the effectiveness of steroidal contraceptives, including for a month after stopping; and it warns of serious rash including Stevens-Johnson Syndrome, so any rash means calling your prescriber.
What favours Vyvanse (lisdexamfetamine)
Vyvanse is the labelled option for ADHD in adults and children 6 and older, and for moderate-to-severe binge eating disorder in adults. Taken once in the morning; afternoon doses are avoided because of insomnia. Before starting, the label directs an assessment for cardiac disease and a check for tics or Tourette's syndrome.
A Schedule II prodrug of amphetamine, it is contraindicated with MAO inhibitors, including within 14 days of stopping one, because of hypertensive crisis. With serotonergic drugs it raises serotonin syndrome risk. It is not indicated for weight loss.
Bottom line
Most "modafinil vs Vyvanse" comparisons quietly assume you are choosing a cognitive enhancer. The labels do not support that framing. One is a wakefulness-promoting agent for sleep disorders; the other is an amphetamine prodrug for ADHD and binge eating disorder. If you have narcolepsy, sleep apnea or shift-work sleepiness, the comparison is not close. If you have ADHD, it is not close in the other direction.
Both are controlled substances, both labels tell prescribers to weigh cardiac disease, and both tell them to watch for misuse. Neither is a study aid, and neither is safe to source outside a prescription.
