These are the same amphetamine salts in two release forms, not two different drugs. The choice is mostly about how long each dose lasts, and it belongs to your prescriber.

The key difference

Both products are the same mixed amphetamine salts (dextroamphetamine and amphetamine). The FDA labels differ in release: immediate-release Adderall is taken in divided doses through the day; Adderall XR is one capsule taken once daily in the morning.

One labelled difference that matters beyond timing: immediate-release Adderall is FDA-approved for both ADHD and narcolepsy, while Adderall XR is approved for ADHD only.

Both carry the same boxed warning for abuse, misuse and addiction, which can lead to overdose and death. Both also warn that sudden death has been reported in patients with structural cardiac abnormalities, and both are contraindicated with an MAOI (including linezolid or IV methylene blue) within 14 days, because of the risk of a hypertensive crisis.

What favours Adderall (mixed amphetamine salts, immediate-release)

A prescriber may reach for immediate-release Adderall when narcolepsy is the diagnosis, since that is an approved use on the IR label but not on the XR label, or when they want to tailor dose timing across the day using divided doses.

What favours Adderall XR (mixed amphetamine salts, extended-release)

For ADHD, Adderall XR is built around a single morning capsule that covers the day, which some people prefer over remembering a second dose. The capsule can also be opened and sprinkled on applesauce for someone who cannot swallow it whole.

Bottom line

We rate drugs on their FDA recall record and sell neither these medicines nor coupons, so we can say plainly what a seller might not: for ADHD these two are the same molecule, and the honest question is whether you want once-daily coverage or divided dosing.
